Please provide inputs or any document which will help in troubleshooting the issue.

None...you need to understand concepts like how CfgServer and URS interacts.
Read logs and understand the why of each result on your URS logs

in your strategy put in a ********TARGET SKILL PRINT STATEMENT********* at the skill targeting block to make it easier to see and find this in your URS logs to see what is being targeted.
