Title: Invalid Conn Id

Please help!
Whenever the user is on a call and then the user needs to hold that call and transfer it but after the incoming icon in the task bar flashes, the user is trying hold the call and transfer but it does work.
Please suggest me some points to debug. This issue is random, this happens to only couple of users in prod and i am not sure what wrong and where to debug.
